National Disability Insurance Scheme (NDIS)


What is the NDIS?

The National Disability Insurance Scheme (NDIS) provides funding for people with disability to obtain individualised support for an improved quality of life. The NDIS funds for assistance or products that help a person in their daily life and help them participate in the community and reach their goals.

Am I eligible for the NDIS?

The NDIS will provide all Australians with a permanent and significant disability, aged under 65, funding to access reasonable and necessary supports tailored to their individual needs.

How to apply for the NDIS?

If you meet the NDIS eligibility criteria and would like to become a participant, you can either:

  • call NDIS on 1800 800110 and ask to make an Access Request
  • complete and submit the Access Request Form and send by email to NAT@ndis.gov.au
  • contact your local NDIA or Local Area Coordinator (LAC) Partner office

How can Alzheimer’s WA help you?

Alzheimer’s WA sees you before the dementia. Every person’s experience with dementia is different, and we provide tailored services to help you through your journey with dementia.

As an NDIS Registered Provider, we are regulated by the NDIS Quality and Safeguards Commission to provide quality and safe NDIS supports and services to our NDIS Participants.

Support Coordination (Level 1 and 2)

Level 1 Support connection – This support is to build your ability to connect with informal, community and funded supports enabling you to get the most out of your plan and pursue your goals.

Level 2 Coordination of supports: This support will assist you to build the skills you need to understand, implement and use your plan. A support coordinator will work with you to ensure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and be included in your community.

Depending on your individual goals, plan objectives and aspirations you may receive funding for these supports in your plan.

NDIS Services 

We offer the following NDIS Services:

 

Occupational Therapy

Occupational therapists are qualified health professionals who specialise in enabling people with injury or disability, including dementia, to continue participating in everyday life and to retain their existing abilities for as long as possible. Whether you need help with managing your appointments, independent showering and dressing or preparing meals, our occupational therapists will work with you in your own home and provide you with practical strategies to complete tasks independently and safely.

 

Short Term Accommodation (Short Stay Respite Services)

Alzheimer’s WA provides short term accommodation at our dementia specialist households.

This service provides an opportunity for carers to have a break while the person living with dementia enjoys a few days in our dementia specialist care houses. We are focused on ensuring that a stay at one of our houses enhances wellbeing and provides the best experience for the house guest.

Sometimes a short stay away from home:

  • gives you the chance to try new things
  • can be a place to make new friends or develop new skills
  • may help to maintain your current living situation by giving your informal supports a break
  • may suit your needs if your usual support network isn’t available for a short period.

Our Short Term Accommodation includes:

  • personal care
  • accommodation
  • food
  • activities

Community Supports

Individual community supports

Alzheimer’s WA can provide individual community supports such as personal and social supports in your home and in the community to support you to continue living independently and engaging in the activities of your choice.

  • Personal care supports relate to assistance with daily personal activities including assistance with, or supervision of, personal tasks of daily life.
  • Social supports can assist you with Social and Community Participation (e.g. supports to enable you to engage in social or recreational activities)

These services are flexible to meet your individual needs and can be adjusted to suit you according to your needs and wishes.

 

Individual overnight respite (in your home)

We can also provide overnight respite in your home, if you would prefer to stay in a familiar environment, if your usual support network isn’t available for a short period.

 

Day Centres

In addition, our dementia specialist households in Perth, Mandurah and Albany provide an evidence-based environment that seeks to maximise wellbeing in the person living with dementia. Providing a wide range of meaningful activities based on the person’s strengths, identity and interests, the houses provide meaningful engagement, stimulation and friendship in specially designed dementia enabling environments.

Day Centre locations:

  • Mary Chester House (Shenton Park): Operates Monday – Saturday between 10.00am and 3.00pm
  • Ella’s House (Mandurah): Operates Monday – Friday between 10.00am and 3.00pm
  • Hawthorn House (Albany): Operates Monday – Saturday between 10.00am and 3.00pm.

Social Support Groups

Friendship clubs and social support groups provide a safe environment for people living with dementia to socialise, remain active in the community and maintain relationships.

An important aspect of the groups is that members have input into activities to ensure a sense of belonging and ownership.

An Alzheimer’s WA staff member meets with all new members to find out their interests before they commence a group. As well as providing benefits for those living with dementia, our services also allow carers and loved ones some time to themselves.

We work closely with you to understand your situation and provide the quality support that you need.

NDIS Fee Schedule

Alzheimer’s WA charges the Temporary Transformation Payment (TTP) rates where applicable. The TTP is a conditional loading fee that assists registered providers to continue transforming their business in the move towards a more competitive marketplace.
