Finding the right level of specialist dementia care in the home can be challenging; and navigating the age care support process can be confusing, overwhelming and at times stressful. This is why, as dementia care experts, we are here to help you every step of the way. We know, first-hand, just how important it is for our clients to receive high-quality support not just in the community but in the home as well.
Our dedicated team at Alzheimer’s WA are here to offer both personalized community support (Commonwealth Home Support Program) and in-home care (Home Care packages) to our existing and new clients who would like to join us now. Our purpose is to keep you or your loved one in your own home for as long as possible with the necessary supports in place.
Our aim is to help you or your family member stay connected and in your community, at home for as long as possible. Home-based care is our preference as it keeps you connected to your community, we won't recommend the move into residential care until it is in your best interests to do so.
You are an individual which is why our community groups, Home Care packages, respite care and the activities in our day houses are personalised to you, and what you would like to do. At Alzheimer's WA, we focus on person-centred care.
We are experts in dementia care, from our experienced customer service officers to our supportive Home Care Package coordinators, our in-house consulting Education team and our support workers.
We can guide you every step of the way, including discussing the services that are available to you now. We recommend you apply for your ACAT assessment as soon as possible. We can guide you through the steps or even submit your booking for the assessment for you.

Your Package is Government-subsidized and means-tested. The total amount of a Home Care package comprises of the Australian Government subsidy and your contribution.
The Government payment differs depending on the level of in-home care support (ranging from Home Care Package level 1 – 4).
The Government contribution is paid directly to us, your Home Care Provider, and we will work with you to personalize your Home Care Package to meet your specific needs, goals and preferences.
Your home care package is a Government subsidy designed to support you in your own home. There are four levels of home care package available. Level 3 and 4 are often referred to as high level packages and are the most common for people living with dementia.
The income-tested fee is determined by the Government and will be based on your individual income.
The basic daily fee provides an opportunity for you to contribute to your care. A maximum fee is set by the Government, and should you choose to contribute a fee when designing your care plan, the extra funds will allow you to access more services under your plan. This option can be discussed with your care manager when designing your plan.
If the level of your package does not meet all of your needs, you can choose to top up your package.
We can guide and support you every step of the way, including discussing with you or your carer the community
and immediate respite support options available to you.
Your ACAT assessor will meet with you in your home to discuss your needs. At this time, they will complete a comprehensive assessment. This assessment will give them an idea of what support you might need to return home or maintain your independence.
The assessor may also refer you to local providers in your area, however, this choice of provider is entirely up to you. In addition, they may also give you referral codes for costs of services so that you can gain an understanding of the possible services available to you.
Following your ACAT assessment, you will be advised by letter if you are approved for a home care package. The letter will also inform you of the level of your home care package. For more information about package levels please visit My Aged Care.
Once approved, you will be placed on a waiting list in the national queue for a package to be assigned to you. Although there is a wait time, it is advisable to start researching home care package providers to ensure a smooth process when you are assigned a package.

Once a home care package is available, you will be advised by letter that you have been assigned a package.
This letter will include a deadline for you to sign up for a package. It is possible to get an extension to this deadline, however if you do not sign an agreement with a home care provider before this date, you will go back in the national queue.
Alzheimer’s WA is a dementia specialist home care package provider in Western Australia. We aim to help you to remain living at home for as long as possible.
Our dementia specialist home care package coordinator will meet with you in person to discuss you & your family’s needs and goals. Our coordinator will guide you every step of the way through the home care package process. From assisting you through the process of signing your home care agreement to designing a personalised plan that meets your needs, goals and ensures your wellbeing.

Your in-home dementia care team is determined by your own personalised care package. This package is designed by you and guided by your needs & requirements. Your team could include support workers, domestic assistants, gardeners, allied staff, podiatrists, dietitians speech pathologists, nurses and more.
We aim to keep your team as consistent as possible, this will enable you to get to know your team and for your team to get to know you.
Your in-home care plan is flexible and can be adjusted to suit you, your needs and goals.

We offer a range of services to support you, your needs and your goals; From providing a dementia-trained support worker – one who shares your sense of humour and a love of painting, for example. To a domestic assistant who can help maintain your home through to a physiotherapist, dietitian and podiatrist to ensure that you stay, independent healthy and active.
