Am I eligible for the NDIS?

If you are an Australian citizen, are under the age of 65 and are living with younger onset dementia you may be eligible for the NDIS.

What happens if I am not eligible?

Other government and community services such as My Aged Care, Alzheimer’s WA and Carers WA may be able to assist. You can test your eligibility for the scheme as many times as you like.

How do I apply for the NDIS?

You will need to complete an Access Request Form and a Supporting Evidence Form. Your GP or Alzheimer’s WA can help you complete these forms.

To access the forms you can

  • Call NDIS on 1800 800 110, or
  • Download the forms from the NDIS website, or
  • Email nat@ndis.gov.au and ask for the forms to be mailed to you.
