Assistant Coordinator

Alzheimer’s WA are seeking an experienced and passionate assistant coordinator on a full-time basis to join our team at Hawthorn House in Albany.

The assistant coordinator role reports to the House manager and focuses on coordination and development of service delivery. This role is the primary contact for clients, families, advocates and stakeholders and is responsible for ensuring appropriate services and funding utilization are delivered to clients in line with their funding including HCP, CHSP and fee for service.

Duties of this position include, but are not limited to:

  • provide consistent, professional communication and accurate telephone support/information to clients, support workers and office-based colleagues;
    assist in preparation of orientation kits and welcome packs for new staff and clients;
  • assist with service scheduling as required;
  • update client files as required;
  • develop understanding of support worker availability, flexibility and suitability for respite services and inputs and maintains this information in the client management system;
    complete driving assessments with support workers;
  • assist with maintaining accurate service provision details to ensure accurate invoicing and reporting;
  • assist coordinators with invoicing and client purchases;
  • assist with credit card reconciliation;
  • support on-call coordination on a weekly rotation (outside ordinary hours including weekends);
  • report any incident, accident, injury, hit or near miss according to Alzheimer’s WA policy and timeframes;
  • liaise with support workers and carers regarding client services as requested by manager/coordinators;
  • provides telephone support and information to clients and support workers in relation to day-to-day services; and
  • efficient time management to achieve maximum chargeable hours.

Essential Criteria:

  • relevant Certificate lll or IV in Aged Care or Individual Support;
  • intermediate working knowledge of Microsoft Office applications;
  • ability to prioritise, monitor and coordinate workload to meet varying deadlines;
  • NDIS Workers Screening Check;
  • unrestricted working rights (in Australia);
  • fully vaccinated against COVID-19
  • Current WA Driver’s Licence; and
  • Commitment and understanding of Alzheimer’s WA’s Vision, Purpose, Philosophy and Values.

Desirable Criteria:

  • previous experience in aged care;
